Kerry Parents and Friends Association offers an outreach support service to people throughout Kerry. This service is well established and making its presence felt in communities throughout the county, stretching from Ballybunion to Kenmare to Valentia.
Outreach is offered to people not residing in a residential house, but accessing the service from home in their local community. The service is provided on an hourly basis, over a seven-day period depending on the person’s needs and what is funded by the HSE. This new type of service attempts to meet unmet need, where people require supports in their locality.
Outreach support services focus on function skills, health and medical management, community inclusion and integration. A person centred plan is developed (PCP), outlining the goals and objectives of the person and this is implemented on a one to one basis in the community.
